Patient Experience Specialist

About the role

The Patient Experience Specialist is the first point of contact for patients assigned to Duo Health, a medical group focused on chronic kidney disease. Based in San Juan, the role blends remote outreach with community‑based engagement to guide patients through onboarding and ongoing care.

Key responsibilities

• Conduct outbound calls to introduce Duo Health services and build awareness.
• Serve as the initial point of contact, establishing trust with patients and caregivers.
• Schedule initial visits with care team providers.
• Follow up consistently with patients who are unreachable or hesitant.
• Document all interactions accurately in the EMR and internal tracking systems.
• Collaborate with interdisciplinary care teams to ensure smooth patient onboarding.
• Meet or exceed patient engagement targets.
• Participate in in‑person community outreach events throughout San Juan.
