Reabilitar em Casa is seeking a part‑time hybrid Nurse to deliver personalized nursing care in patients' homes across Cascais. The role combines on‑site visits with remote tasks, allowing flexibility while ensuring high‑quality rehabilitation support.

Key responsibilities

• Provide individualized nursing care at patients' homes, including treatment administration and health assessments.
• Monitor vital signs, track patient progress, and update health records accurately.
• Collaborate with multidisciplinary teams to develop and adjust care plans.
• Communicate effectively with patients, families, and healthcare professionals.
• Perform selected remote duties such as documentation and coordination with the care team.

Required profile

• Degree in Nursing with a valid professional license.
• Experience in home‑based care, rehabilitation, or similar settings is advantageous.
• Strong organizational abilities and attention to detail for accurate record‑keeping.
• Excellent communication and interpersonal skills.
