Enfermero/a HHA – Home Health Nurse

About the role

The HHA Registered Nurse supports the daily operations of the Clinical Services Department for Home Care, providing comprehensive nursing assessments, documentation, and patient care for Best Option Healthcare clients.

Key responsibilities

• Conduct initial, follow‑up, and recertification assessments for assigned patients using clinical judgment and nursing standards.
• Complete OASIS documentation for all assigned patients in a timely manner.
• Measure, analyze, and record vital signs, taking appropriate actions and notifying the primary physician as needed.
• Develop individualized care plans and coordinate interdisciplinary interventions.
• Educate patients and families on admission processes, rights, HIPAA, fall prevention, and emergency plans.
• Maintain accurate physical and electronic patient records, documenting activities, problems, and home‑based needs.
• Perform aseptic catheter insertion and changes, as well as nasogastric tube placement and feeding management, providing education to avoid infections or complications.
• Participate in case discussions, interdisciplinary meetings, and peer consultations.

Required profile

• Graduated Registered Nurse with experience in home health or clinical services.
• Strong critical thinking and clinical decision‑making abilities.
• Ability to work independently while maintaining constant communication with the care team.
• Commitment to aseptic techniques, patient safety, and regulatory compliance.
