Remote Social Care Screener

About the role

We are seeking motivated and compassionate remote Social Care Screeners to conduct standardized screenings for a Medicaid 1115‑Waiver Social Care Network in New York. This role supports members by identifying health‑related social needs and connecting them to appropriate resources.

Key responsibilities

• Conduct standardized social care screenings with members.
• Identify health‑related social needs and accurately document findings.
• Maintain confidentiality and comply with HIPAA and program requirements.
• Communicate professionally and empathetically with individuals from diverse backgrounds.
• Ensure timely and accurate completion of assigned screenings.

Required profile

• Minimum 1 year of experience in social care, healthcare, care coordination, case management, community outreach, patient navigation, or similar screening work.
• Strong communication and interpersonal abilities.
• Excellent organizational skills and attention to detail.
• Comfortable using computers and web‑based documentation systems.
• Ability to work independently while meeting productivity and quality expectations.
