Community Outreach Technician

About the role

The Community Outreach Technician supports the case‑management process for health‑insurance policyholders by addressing non‑clinical needs. The role involves conducting assessments, developing individualized care plans, and coordinating community‑based services in line with policyholder preferences and available resources.

Key responsibilities

• Process referrals from Care Management staff, schedule calls and home visits, and document assessments in electronic filing applications.
• Evaluate psychosocial and physical health status of policyholders within 30 days of referral, creating individualized care plans.
• Coordinate non‑clinical services such as transportation, home food delivery, medical equipment acquisition, housekeeping, caregiver respite, and recreation alternatives.
• Perform home visits for readmission prevention, transition of care, and re‑establish contact for Complex Care Management programs.
• Maintain an updated database of community resources and guide policyholders, families, and caregivers on available options.
• Document updates, close care plans, and report metrics according to operational and compliance guidelines.

Required profile

• Associate’s degree or 60‑64 university credits in Health, Social Sciences, or related fields.
• At least one year of customer‑service experience, preferably in the health‑insurance industry.
• Valid driver’s license for the Commonwealth of Puerto Rico and ability to travel throughout the island.
• Intermediate proficiency in Spanish and English (written and verbal).
