About the role

Join Sword’s AI‑driven health platform as a Care Coordinator, supporting members through digital physical therapy programs. You will be the primary point of contact, ensuring a seamless, empathetic experience from enrollment to program completion.

Key responsibilities

• Provide exceptional, solution‑focused customer support across multiple channels.
• Drive member engagement by guiding participants through the virtual health program.
• Collaborate with Physical Therapists to address member needs and act as a trusted resource.
• Establish personal connections with members and tailor communications to individual preferences.
• Leverage internal tools and technology to answer questions accurately and resolve challenges.
• Participate in outreach campaigns to encourage continued progress.

Required profile

• Excellent communication and customer‑service abilities.
• Highly organized with strong multitasking and prioritisation skills.
• Ability to work independently and as part of a cross‑functional team.
• Background in healthcare or customer service is preferred but not mandatory.
• Demonstrated proficiency with AI tools at a basic (Level 1) usage level.
