RN Clinical Case Manager – Kidney Management

About the role

The RN Clinical Case Manager will join Healthmap Solutions’ Kidney Management Program in San Juan, PR. This role focuses on building strong relationships with physicians, their staff, and Healthmap members to improve kidney health outcomes through coordinated care and performance improvement activities.

Key responsibilities

• Act as liaison between Healthmap, provider practices, and members to ensure engagement and program success.
• Develop new provider and partner relationships to enhance member engagement and manage an assigned caseload.
• Identify and act on opportunities to improve health outcomes using provider‑specific data and best‑practice education.
• Educate provider offices on chronic kidney disease, ESRD, and renal replacement therapies.
• Partner with physicians to identify members who would benefit from care navigation, conduct outreach, and engage them in the program.
• Advocate for members, connect them with community resources, and coordinate with their care team to meet individualized goals.
• Maintain thorough documentation of all interactions in compliance with NCQA and HIPAA standards.

Required profile

• Associate, diploma, or bachelor’s degree in nursing.
• Active, unrestricted RN license.
• Basic Life Support (BLS) certification.
• Advanced Cardiovascular Life Support (ACLS) certification.
