Inpatient Clinical Nurse

About the role

Join Triple‑S as an Inpatient Clinical Nurse and play a pivotal role in delivering high‑quality, affordable health care to over one million Puerto Rican consumers. You will work directly with admitted patients, coordinating multidisciplinary care and ensuring smooth transitions from hospital to community settings.

Key responsibilities

• Visit participating hospitals and health centers to verify that admission records meet established clinical criteria and document audit findings.
• Enroll identified enrollees in the Inpatient Clinical Program and complete admission assessments within the first 72 working hours.
• Develop and monitor individualized care plans, coordinating required services at least 48 hours before discharge.
• Perform discharge assessments on the day of discharge or within 24 hours, and arrange primary‑care follow‑up within seven days.
• Collaborate with social workers and discharge planners to ensure seamless service coordination and refer cases to cost‑containment programs as appropriate.

Required profile

• Bachelor’s Degree in Nursing (BD) with 1‑3 years of clinical experience, preferably in a hospital setting.
• Active, unrestricted nursing license in Puerto Rico and membership in the College of Nursing Professionals of PR.
• Demonstrated ability to collect, analyze, and interpret inpatient data for decision‑making support.
• Strong communication skills to interact with physicians, utilization review managers, and hospital staff.
