Enfermero/a HHA – Atención domiciliaria

About the role

The Graduate Nurse (Enfermero/a Graduado) will support daily operations of the Clinical Services Department for Home Care. The role involves comprehensive patient assessments, documentation, and delivery of nursing services to Best Option Healthcare’s home‑care patients.

Key responsibilities

• Perform initial, follow‑up, and recertification assessments using clinical judgment and nursing standards.
• Complete OASIS documentation for all assigned patients in a timely manner.
• Measure, analyze, and record vital signs; take necessary actions and notify the primary physician.
• Develop individualized care and treatment plans based on patient, family, and community needs.
• Coordinate interdisciplinary interventions and integrate additional disciplines as required.
• Educate patients and families on admission processes, rights, HIPAA, fall prevention, emergency plans, and recovery strategies.
• Maintain physical and electronic patient records, documenting activities, problems, and home‑based needs.
• Adhere to aseptic, clean, or sterile protocols and Best Option Healthcare’s established procedures.
• Provide catheter and nasogastric tube insertion, care, and replacement, educating patients and families to prevent infections or complications.
• Assess and manage skin lesions using aseptic techniques.

Required profile

• Graduate Registered Nurse (RN) qualified to practice in a home‑health setting.
• Strong clinical judgment and ability to document assessments accurately.
• Effective communication skills for interdisciplinary collaboration and patient education.
