Clinical Pharmacist – Call Center (San Juan, PR)

About the role

Optum is seeking a Clinical Pharmacist to join its Call Center team in San Juan, PR. The pharmacist will conduct telephonic medication reviews, support adherence, and help members manage disease states while collaborating with primary care providers.

Key responsibilities

• Engage members by phone and schedule pharmacist consultation appointments.
• Maintain HIPAA compliance and protect patient confidentiality.
• Use prescription claims data to generate clinical recommendations following Medication Therapy Management (MTM) and Retrospective Drug Utilization Review (RDR) protocols.
• Assess medication regimens for unnecessary, ineffective, or unsafe drugs and provide education on therapy and disease states.
• Document care notes and recommendations clearly and concisely.
• Stay current with emerging clinical trends and participate in departmental meetings.
• Meet productivity targets and support special projects as needed.

Required profile

• Active, unrestricted Pharmacist license or Pharmacist Intern (Pharm D) license in Puerto Rico.
• Experience working independently within a cross‑functional team.
• Self‑directed with strong organizational abilities.
• English proficiency assessment required.
