Advanced Practice Provider (NP/PA) – Bilingual English/Spanish

About the role

Duo Health is seeking a bilingual Nurse Practitioner or Physician Assistant to join its interdisciplinary care team. You will provide high‑touch, value‑based care to patients with chronic kidney disease (CKD) stages 3b‑5 and end‑stage renal disease (ESRD) across the Aguadilla area, delivering services in patients’ homes, community sites, and via telehealth.

Key responsibilities

• Manage an assigned panel of CKD/ESRD patients in collaboration with physicians, nurses, social workers, and patient navigators.
• Conduct comprehensive history and physical examinations, order and interpret laboratory and diagnostic tests, and prescribe medications within regulatory limits.
• Educate patients and caregivers on medication use, disease management, and treatment plans.
• Develop and implement individualized care plans, perform structured assessments, and coordinate referrals to medical, surgical, or behavioral health specialists.
• Facilitate care transitions across emergency departments, hospitals, skilled nursing facilities, home‑health agencies, hospice, and community organizations.
• Participate in regular care‑team huddles and be available for after‑hours on‑call rotation.

Required profile

• Current NP or PA licensure with the ability to prescribe medications.
• Bilingual proficiency in English and Spanish.
• Experience caring for patients with CKD stages 3b‑5 and ESRD.
• Willingness to travel to patient homes and community locations.
• Availability to work Monday‑Friday, 8:00 am‑5:00 pm, with paid on‑call duties.
